Are you sure Firefox is shutting down when you close it? If there are any firefox.exe processes in task manager it will keep using the same loaded (safe-mode) profile.

Firefox shortcut doesn't have -safe-mode on it, does it?
I reckon, bite the bullet (backup) and delete your Firefox profile.

Unpin it and try running firefox.exe using winkey+R and then re-pin that running instance?
